CARE FACTOR
This Patient Support Program (PSP) helps people with Haemophilia prescribed ADVATE® (octocog alfa), ADYNOVATE® (rurioctocog alfa pegol) or FEIBA NF® (Factor VIII inhibitor bypassing fraction).
The program provides patients prescribed a Takeda haemophilia product access to home delivery of medical supplies required for their ongoing haemophilia treatment.

Register yourself
Complete your registration by printing and completing the HCP registration form, before scanning/taking a photo and emailing to support@carefactor.com.au
Enrol patients
Patients eligible for this service are those prescribed ADVATE, ADYNOVATE or FEIBA NF. Please print the enrolment form below, including ensuring your patient has read and understood the program terms and conditions. Upon completion, scan/take a photo and email to support@carefactor.com.au
Ongoing medical supplies delivery
Once your registration and patient enrolment has been confirmed, automatic medical supplies delivery orders will be set up for your patient. If you require additional supplies not included in your automatic orders, you may request additional supplies once a month by calling or emailing the program at support@carefactor.com.au
